The Dental Safari Company will be coming to our school on Monday, March 9, 2020 and will be offering services to every student within our school. All Illinois children in Kindergarten, Grade 2, Grade 6 and Grade 9 are required to have an oral health examination. Examinations must be performed by a licensed dentist, and he/she must sign the IL Proof of School Dental Examination form. Each child is required to present proof of examination by a dentist prior to May 15th. Consent forms will be going home with Grade School students this week and forms will be available for pick up in the High School office. To take part in this service, please complete and return the consent form no later than Friday, February 28th. You may also visit www.dentalsafariforms.com to register online. All Children Are Eligible:
· Children with a Medicaid / All Kids Identification number.
· Children who are on free or reduced lunch program.
· Children with private insurance (provide information).
· Child with cash payment (cash or check) ($75 – includes exam, cleaning, fluoride, sealants as needed).
