The Dental Safari Company will be coming to our school on Friday March 5, 2021 and will be
offering services to every student within our school. DENTAL SAFARI COMPANY is a fully licensed, professional company that believes dental wellness begins with a 3-part approach: Education, Treatment, and Prevention. On March 5th, your child/children will receive a visual exam (no x-rays) by a licensed dentist, a cleaning, Fluoride treatment, and sealants as needed.
All Children Are Eligible:
- Children with a Medicaid / All Kids Identification number
- Children who are on free or reduced lunch program
- Children with private insurance (provide information)
- Child with cash payment (cash or check)
- ($75 – includes exam, cleaning, fluoride, sealants as needed)
All Illinois children in Kindergarten, grade 2, grade 6 and grade 9 are required to have an oral
health examination. Examinations must be performed by a licensed dentist, and he/she must
sign the IL Proof of School Dental Examination form. Each child is required to present proof of
examination by a dentist prior to May 15th . The exam performed by Dental Safari does meet
state requirements and is a great opportunity. Following your child’s treatment, a letter will be
sent home indicating the treatment your child received during the visit as well as follow-up
treatment he/she may need.
